Healthy Rewards Program

What is the Healthy Rewards Program?

The Healthy Rewards Program rewards you for taking small steps that help you for going to the doctor. You’ll receive a reloadable prepaid Visa® debit card, gift card or e-gift card for completing certain healthy activities and milestones, like:

  • Seeing your PCP
  • Getting all of the recommended well-child check-ups for your children
  • Making sure you get your yearly well women screenings
  • Going to all of your prenatal visits, if you’re pregnant
  • Working with your providers to manage chronic illness (like Diabetes)

The more activities your child completes, the more rewards your child can earn!

Staywell Healthy Rewards

The Healthy Behaviors Program runs on a calendar year; the evaluation period of this report includes the end of the 2018 calendar year program and the beginning of the 2019 calendar year program.

We have updated the Healthy Behaviors Program! The correct incentives are listed below. Please use this chart instead of the one in your member handbook.

Smoking Cessation Program

Attend and finish a program to stop smoking

Reward: $10

Weight Loss Program

Attend and complete an approved weight loss program

Reward: $10

Substance Abuse Program

Attend and complete an alcohol or substance abuse program. 

Reward: $10

Health Survey

Complete a Health Survey form with the health plan within 60 days of enrollment.

Reward: $10

Initial PCP Visit

Visit a PCP within 90 days of enrollment.

Reward: $10

Newborn well-child visit

Take your child to wellness visits from birth (0 months) to 15 months of age. You will get a reward for each visit. This is up to six visits.

Reward: $10 per visit for a total of $60

Annual Child Visit

Take your child for annual check-ups between 3 and 6 years old

Reward: $20

Annual Adolescent Visit

Take your child for annual check-ups between ages 7 and 20.

Reward: $20


Prenatal Care Visits

Go to your prenatal visit during your firs trimester or within 42 days of enrolling with Staywell

Reward: $20

Members who complete a second visit will get their choice of a stroller, portable play pen, car seat, or six (6) packs of diapers

Postpartum Care Visit

Visit your doctor 7 – 84 days after your baby is born

Reward: $20

Annual Eye Exam

Complete an eye exam. For members ages 13 - 20 years old with diabetes

Reward: $20

Diabetic A1C Blood Test

Complete an annual HbA1C lab test. For members ages 18 - 75 with diabetes. 

Reward: $20 

Blood Pressure Control

Complete a blood pressure control check. For members ages 18 - 75 with diabetes

Reward: $20 

Mammogram Screening

Visit your doctor for an annual mammogram screening. For members ages 50 – 74

Reward: $20

Cervical Cancer Screening

Visit your doctor for an annual cervical cancer screening (pap smear). For members ages 21 – 64

Reward: $20

Chlamydia Screening

Visit your doctor for an annual chlamydia screening. For members ages 16 – 24

Reward: $20

Annual Adult Health Screening

Visit your doctor for an annual adult wellness visit. For members ages 20 and over

Reward: $20
