Sign Data Sharing Agreement to Exchange Patient Health Information

This is a follow-up to provider update 22-976, Sign Attestation by January 31, 2023, to Exchange Patient Health Information distributed on November 10, 2022.

The Data Sharing Agreement can be found at the Data Exchange Framework’s (DxF) portal. To sign the agreement, go to signdxf.powerappsportals.com.

What's next

After signing the Data Sharing Agreement, you must execute the Data Exchange Framework with an entity that delivers data exchange or by using your organization’s own technology. This will promote the safe and secure exchange of patient health and social services information.

Participating entities include:

  • General acute care hospitals
  • Physician organization and medical groups
  • Skilled nursing facilities
  • Clinical laboratories
  • Acute psychiatric hospitals
