Children's Medical Services Health Plan: Medicaid

CMS Health Plan for Medicaid is is designed for children under age 21 who:

  • Are eligible for Medicaid, and
  • Have special health care needs that require both preventive and ongoing care

These children may need extensive health care services. This could mean working with several different doctors at once. It might also require working with state agencies or finding local community resources. It can be tough to keep track of it all. We understand. We’re here to help.

Care Coordination Services

Managing your child's medical issues can be hard. We’ll help guide your family through the health care system.

Our team of nurses and social workers will work with you and your child's doctors. They’ll help to identify your child’s needs. And they’ll help you to find the resources available to meet them.

We’ll help you schedule things like doctor visits, medical tests, and procedures. And, we can help you get behavioral, developmental, and social services, too.

Children's Medical Services Health Plan Documents

CMS Health Plan will begin monitoring engagement and utilization of its Healthy Behavior Program once it is approved and implemented.

Florida has a special program for children with a serious illness. Partners in Care: Together for Kids (PIC:TFK) helps your child and family get special care and keep up your quality of life.
