Staywell Health Plan (Staywell) has been selected by the Agency for Health Care Administration (Agency) to participate in the SMMC Housing Assistance Pilot in Region 5 and Region 7.
Through the pilot, Staywell will provide permanency related housing supports using evidence-based practices. The goal is to provide additional behavioral health services and supportive housing assistance services for members ages 21 and older with a severe mental illness (SMI), a substance use disorders (SUD), or SMI with co-occurring SUD, who are homeless or at risk of homelessness due to their condition. Ultimately, the goal is to keep these members in sustainable housing through improved supports and to avoid avoidable high cost services through better care coordination, such as potentially preventable hospital events.
The pilot is limited to serving no more than 4,000 Medicaid recipients per State Fiscal Year. The 4,000 slots will be distributed equally among four participating health plans by the Agency.
Staywell’s Housing Assistance Program
Staywell will identify members eligible for participation. Providers can also refer Staywell members by calling 1-866-635-7045. Members are not required to be in a treatment program to be eligible to participate in the housing assistance program.
Each participating Staywell member will have an assigned care manager. The care manager will work closely with the member and providers to coordinate housing assistance services as well as the member’s health plan benefits. The services provided to members in this program are in addition to their current Staywell health plan benefits.
Once enrolled in the program, members will be supported for one year or until they are stable and meet discharge criteria.
Staywell will provide housing assistance through agreements with housing providers and relationships with local housing coalitions.
