2018 'Ohana Health Plan PCP Attestation

The Department of Human Services (DHS) Med-QUEST Division (MQD) is extending its enhanced reimbursement rate through Dec. 31, 2018, for certain primary care services provided by eligible primary care physicians (PCP). Eligible PCPs will receive Medicare rates for certain primary care services. However, this payment and the attestation process will now be completed by ‘Ohana Health Plan.

For purposes of the increased reimbursement, eligible primary care physicians are considered those by ‘Ohana those who:

-Self-attest to practicing in family medicine, internal medicine, pediatric medicine, obstetrics and gynecology, and to subspecialists of those specialties as recognized by the American Board of Medical Specialties (ABMS), American Osteopathic Association (AOA), or American Board of Physician Specialties (ABPS); and,

-Are an in-network provider and have an open panel with ‘Ohana Health Plan.

Please read and sign the following PCP attestation form. We appreciate your cooperation with this matter, and thank you for serving our members.
