As a valued Wellcare DSNP member, we want to ensure you have access to all the benefits you are entitled to.

It’s important you take action when the time comes to renew your Medicaid eligibility. Wellcare partners with Centauri to help members like you reapply for Medicaid and the Medicare Savings Program. Centauri is a health solutions company that focuses on helping make the healthcare system work better for you. Plus, if you are approved, you may be able to receive additional money each month in your Social Security check, based on your current income and resource levels.

Renewing your Medicaid eligibility is as easy as 1-2-3.

  1. You should receive a letter from your state Medicaid agency a few months before your Medicaid anniversary that explains how to verify your Medicaid eligibility.

If you are eligible for Medicaid, you can continue your coverage (or choose another plan). If you are no longer eligible for Medicaid, just give us a call at 1-833-444-9088 (TTY 711) Monday-Sunday, 9:00 a.m. to 8 p.m., and we would be happy to walk you through your coverage options.

  2. If you don’t receive your letter or are unsure of your anniversary date, contact your state Medicaid Agency.

Please call Centauri toll-free Monday-Friday 9:00 AM – 7:30 PM EST. at 1-(877)-236-4471 (TTY 711) for more information. Their caring and highly trained associates are ready to help you today.

Why is this important?

If you don’t verify your Medicaid eligibility on time, you will lose your Medicaid and Medicare Dual Special Needs plan benefits. Your health is important to us. We want to help in any way we can to ensure you continue to get the coverage you need and deserve.
