Review these helpful instructions to learn how to access your plan details and documents, including the Annual Notice of Change, Evidence of Coverage, Summary of Benefits, and Drug List (Formulary). 

To get started, select your state from the drop-down on the Member homepage

How to Use the Find My Plan Tool

  • From the “My plan is” dropdown, select “Prescription Drug Plans”
  • Enter your zip code and select “Search”
  • Locate your plan from the list of plans and select “Go to my plan details." You will be taken to the Plan Overview page.

How to Locate Plan Details and Documents from the Plan Overview Page

  • Plan Documents: locate your Annual Notice of Change, Summary of Benefits, and Evidence of Coverage documents in the “Plan Specific Documents” section of the page. You can also locate the Appointment of Representative and Pharmacy Direct Member Reimbursement forms in the "Related Materials" section of the page.
  • Drug List (Formulary) and Prescription Drug Claim Form: You can locate your plan’s Drug List (Formulary) and Prescription Drug Claim Form by selecting “Pharmacy” from the right sidebar. From the “Pharmacy” drop-down menu, select the “Drug List (Formulary) and Other Documents” page.
  • Pharmacy Resources: Locate additional Pharmacy information such as Medication Guide, Medicare Extra Help, Specialty Pharmacy, Mail order Service, and Transition Policy by selecting “Pharmacy” from the right sidebar. From the “Pharmacy” drop-down menu, select the additional Pharmacy information page that you’re interested in reviewing.
