The Dual Eligible Special Needs Plan (DSNP) Health Home Program is a set of services available to eligible Wellcare members who:
- Actively participate in Medicaid Health Home Program;
- Have Medicaid and Medicare;
- Have a serious chronic condition;
- Are at risk for a serious chronic condition; and
- Have a serious health issue that typically requires more than one service provider
DSNP Health Home services include:
- Comprehensive care management;
- Care coordination and health promotion;
- Transition planning;
- Individual and family support; and
- Referral to relevant community and social support services
