- Online. Choose your plan and complete your application online.
- By Phone. Contact Us and one of our licensed sales and enrollment representatives will help you through the enrollment process.
- By Mail or Fax. Download and print your enrollment application. Once you fill it out, mail or fax it to us. If you need help, Contact Us to speak to a licensed sales agent.
- Use our Need Plan tool to search for and access your plan’s information and documents, including the enrollment application. Once you’ve found your desired plan, select “I want to learn about this plan.” You’ll then be taken to the plan’s overview page. Here you can access the enrollment application, plan information, and other important documents.
